A sustainable approach to Case&Care management in the crossborder region

The aim of CaRe project is the development, regional piloting and evaluation of a superordinate cross-institutional CrM office, which serves as a regional contact point & counseling center for the system & care management of elderly people in need of care and their caring relatives.

The planned activities are:

  • Horizontal coordination activities, kick offs, intermediate meetings and final conference;
  • Communication activities and dissemination of actions and projects results
  • Modeling activities of the regional welfare network system, including analysis of the qualification of personnel and companies, and related assessment and analysis
  • Piloting activities based on a model of Best Practice identified during the project
  • Evaluation of piloting and development of a cross-border communication campaign for its transferability to other regions, as well as for sustainability activities.

The expected outcome are:

  • Development of care management (CrM) concerned with the system & care control, which coordinates and organizes across cases and institutions needs oriented support in social-and health care and demonstrates structural prerequisites for it.
  • Creation of an innovative CrM office that provides sustainable management & coordination of c are quality & cost-efficiency. This leads to a regional change in the program area.
  • Add competences to the existing organizational forms of case management (CsM) in order to keep the balance between humanity and economics.

Type of service

Support in the definition of the communication plan and its subsequent implementation, including the realization of the main communication tools identified by the project (bilingual website, bilingual newsletter, bilingual documentation for the press conference for the launch and conclusion of the project, flyer, roll-up, etc.) and for the organization of specific promotion campaigns to draw the attention on the web, as well as for the drafting of the final communication report.
